Minimum size by code
A powder room is a toilet room, which the International Residential Code does not count as habitable space (R202). The size comes from the clearances: 15 in from the center of the toilet and the sink to a side wall, 30 in center to center, and 21 in in front of each (P2705.1). The ceiling must be at least 6 ft 8 in (R305.1).
With the toilet and the sink on the same wall, that adds up to 15 + 30 + 15 = 60 in of wall. With the fixtures on opposite walls, the room can be narrower but needs more depth.

What designers recommend
The National Kitchen & Bath Association recommends more room than the code: 18 in from the center of the toilet to a wall or fixture, and 30 in of clear floor in front of each fixture.
